Documentation & Letter Requests
Documentation and letters may be available to established patients when clinically appropriate and supported by the medical record.
Emotional Support Animal (ESA) Letter — $129 — requires at least 60 days of established care
Diagnosis Letter — $39 — requires at least 30 days of established care
FMLA Short-Term Leave Application — $129 — requires at least 30 days of established care
FMLA Application Revision — $59
Accommodation Letter — $89 — requires at least 30 days of established care
504/IEP Supporting Documentation — $39 — requires at least 30 days of established care and is provided when clinically appropriate and supported by the medical record. Eligibility for a 504 Plan or IEP and specific educational accommodations are determined by the school or school district.
Please note: Documentation is provided at the treating provider’s clinical discretion. Payment of a documentation fee does not guarantee that a letter, certification, accommodation, or specific recommendation will be issued. Additional evaluation or supporting records may be required.

Start with a comprehensive psychiatric evaluation, then choose the payment option that works best for you: $149 month-to-month membership or $149 per follow-up visit with Pay-As-You-Go care.
Care is provided virtually throughout Florida for eligible patients.

Comprehensive Psychiatric Evaluation
Your self-pay care begins with a comprehensive psychiatric evaluation for $199.
After your initial evaluation, you can choose how you would like to continue your care.
Option 1: Mental Health Membership — $149/Month
Choose our month-to-month membership if you prefer predictable monthly pricing for ongoing psychiatric care.
Your membership begins at your 2-week follow-up and is $149 per month.
Your Membership Includes:

✔️ Medication management and medication adjustments when clinically appropriate
✔️ Supportive, solution-focused therapeutic care integrated into psychiatric visits
✔️ Secure patient portal messaging for non-urgent questions and care coordination

✔️ Continuity of care with the same psychiatric provider
✔️ Convenient telehealth appointments throughout Florida
No long-term contract. Your membership is month-to-month.
If you have no visits for 3 consecutive months, your membership will restart with a $199 comprehensive psychiatric evaluation when you return to care.
Option 2: Pay-As-You-Go — $149 Per Follow-Up Visit

Choose Pay-As-You-Go and pay $149 for each psychiatric follow-up visit.
There is no monthly membership fee and no long-term commitment.
You simply pay for your visit when you need ongoing psychiatric follow-up care.

Need a Superbill?
Self-pay patients with eligible out-of-network insurance benefits may request a superbill after services are rendered.
A superbill contains information you may submit to your insurance company for possible out-of-network reimbursement.
Reimbursement is determined by your insurance plan and is not guaranteed by Sunset Behavioral Health.

The Sunset Behavioral Health Mental Health Membership and Pay-As-You-Go options are not health insurance and do not replace health insurance.
Patients enrolled in Medicare, Medicaid, or other government healthcare programs may be subject to separate federal and program-specific billing requirements.
